Working with Children Forms
We have to have Working with Children clearances in order to run programs where there is any interaction with children/schools etc.
Please assist by processing the paperwork. Any queries see Fiona or Ian. The form is self-explanatory but key section advice to note:
Part 5:
Rotary falls into category 12; Type of employment is Volunteer/Unpaid position; Organisation is Bunbury Leschenault Rotary Club; Role is Member
Part 6:
Employer/Agency representative can be either Fiona Hinds or Ian Telfer. Note whoever you put down in this section must sign under Part 7. Street Address leave blank as we don’t have one and Postal address is PO Box 1067 Bunbury 6231. Notice is to be sent to Fiona Hinds.
Please take your completed and signed form to Australia Post as explained by Fiona. Bring your receipt to Neville and the $10 fee will be reimbursed.
